RITUAL BREATHWORK™ WAIVER + INFORMED CONSENT 

By registering for and participating in this Ritual Breathwork™ event (in-person at Ventura Therapy Collective and/or virtually), I acknowledge and agree that:

Ritual Breathwork™ is a personal growth and self-exploration practice and is not medical care, psychotherapy, or a substitute for professional medical or mental health treatment. I understand that breathwork may involve physical, emotional, and energetic responses, which may include (but are not limited to) tingling, dizziness, trembling, cramping, emotional release, and other intense sensations.

I confirm that I am at least 18 years old and that I am voluntarily choosing to participate. I understand that I am fully responsible for my own health, well-being, and participation, and I agree to listen to my body and modify, pause, or stop at any time.

I understand Ritual Breathwork™ is not recommended for individuals who are pregnant, or who have certain medical or psychiatric conditions including (but not limited to) cardiovascular disease, history of heart attack or stroke, high blood pressure, glaucoma, retinal detachment, osteoporosis, seizures/epilepsy, recent surgery, acute infectious illness, or major psychiatric conditions. If I have any doubt about whether I should participate, I agree to consult my primary care physician prior to attending. If I have asthma, I will bring my inhaler and consult my physician as needed.

I understand that participation involves inherent risks, and I assume all risks of participation, whether known or unknown. To the fullest extent permitted by law, I hereby release and hold harmless Ritual Breathwork™, its owner(s), facilitator(s), assistants, contractors, and venue partner Ventura Therapy Collective, from any and all liability, claims, demands, or causes of action arising out of or related to my participation, including any physical injury, emotional distress, illness, or other adverse effects.

If participating virtually, I confirm I will join from a safe location and will not participate while driving or in any unsafe environment.
